Over half a million usernames and passwords from the video game Battlefield Heroes were hacked and made public by the hacker group "LulzSec" in June 2011 as part of a final data breach dump. The passwords were kept as MD5 hashes without any salt, and many of them could be quickly changed back to plain text.
